To order over the phone, find the products you want to order in the catalog and call 1-866-413-2582, TTY 711. You will need the approved item name (s), item ID (s), your OTC card number and security code, and your shipping address to place the order. Your order will be shipped free of cost thanks to your health plan.
How to use the Humana Healthy Foods Card
Dual-SNPs are for individuals who are entitled to Medicare and who are also eligible for assistance from a state Medicaid program. SNPs are available to anyone who meets the specific eligibility requirements of the plan, which can vary by state and service area.
